Superlift has a 6" lift and I haven't heard anything bad about it.  Not sure how much for the lift specifically, but when you figure in new wheels, tires, installation (unless mechanically inclined) its pricy.  Big ballpark I'm gonna guess $3000-4500.  Maybe, thats a ballpark guess.  I do know that truckperformance.com sells the lift if you are looking for more specific pricing.

BDS makes a 5"
Trailmaster makes 5"
Superlift 6"
they all use leaf over for the back
and i believe rough country makes a kit now,
$3800 for everything wheels, tires, kit, labor,  try to get a good deal for labor if you need it, and buy 4 tires and wheels from same place (ex:4wheel parts.com and they usually give good deal as package, and they mount and balance them for free.

ive heard of problems with the TM and BDS though
